Referrals

Regarding primary care physician referrals and authorizations for HMO Plans and Workers Comp: Please review your insurance policy requirements to determine if you need a referral from your primary care physician or your workers’ compensation claims adjuster prior to your visit. Please call before you arrive to confirm that all needed referrals or authorizations have been received.

Not all companies we work with may be listed here and our insurance providers are subject to change. If your insurance company is not listed, please contact your carrier for more information.

Workers’ Compensation

If you are seeing us for a workers’ compensation related injury, we will need the following information before we can see you:

  • Workers’ compensation insurance company name and billing address
  • Claim number
  • Date of injury
  • Claim adjuster name, phone number, and fax number
  • Employee name, address, and phone number

Please note that our team of patient coordinators will reach out to the claims adjuster or the attorney managing your case to collect all the required medical records for your initial visit. Working with multiple parties can create delays in retrieving your records and scheduling your appointment. We make every effort to call you with updates and appreciate your patience during this process. You may always call our office and speak to a patient coordinator for status on your records and authorization.

Helpful Things to Remember

Please remember to bring your insurance card(s) and ID card to your appointment. Also, alert the office if your insurance information or personal information, such as address, telephone number, etc., has changed and must be updated. You can call the office to confirm that your doctor is a provider under your coverage. Your insurance may require a co-pay for your visit and this should be paid at the time of your appointment.
